Who Can Refer

We accept referrals from:

  • Local authority commissioning teams and social workers

  • NHS hospital discharge and rabblement teams

  • Community mental health and learning disability teams

  • Housing and supported living providers

  • Charities, voluntary organizations, and advocacy services

  • Private clients or families (self-referrals)

Our Referral Process

 Initial Contact

Professionals can refer individuals directly through:

Once received, our team acknowledges every referral within 2 working hours and begins initial triage and assessment.

Needs Assessment

Registered Manager or Clinical Associate (former nurse) reviews the referral details and contacts the referrer or individual to arrange a holistic assessment.
We assess:

  • Personal care and health needs

  • Social circumstances and housing environment

  • Risk factors and safeguarding considerations

  • Preferred care outcomes and support duration

Assessments are completed within 24–48 hours for standard referrals or within 4 hours for urgent hospital discharges.

Care Planning and Agreement

Following assessment:

  • personalized care plan is created and shared securely with all relevant professionals and family members.

  • We align our delivery model with council frameworks, NHS discharge protocols, and CQC requirements.

  • The service start date is confirmed, and the individual’s care file is set up digitally in D C M E (Digital Care Management Environment).

 

Urgent or Crisis Referrals

We understand that situations can change quickly.
Our Rapid Response Service allows us to mobilize support within a few hours in cases of:

  • Unplanned hospital discharge

  • Breakdown of existing care arrangements

  • Emergency respite or overnight support

  • Safeguarding concerns requiring immediate intervention

Our on-call management team operates 24 hours a day, 7 days a week to respond promptly and effectively.

All referrals are handled in accordance with:

  • CQC regulations (Personal Care activity)

  • GDPR and Data Protection Act 2018

  • Safeguarding Adults Policies

  • Information Governance Frameworks
