Assessments are completed within 24–48 hours for standard referrals or within 4 hours for urgent hospital discharges.
Care Planning and Agreement
Following assessment:

We align our delivery model with council frameworks, NHS discharge protocols, and CQC requirements.
The service start date is confirmed, and the individual’s care file is set up digitally in D C M E (Digital Care Management Environment).
