Our Home From Hospital Service provides safe, seamless, and person-centered support that enables adults to return home confidently after a hospital stay. We work closely with hospitals, rabblement teams, discharge coordinators, and community health professionals to ensure a smooth transition, reduce delays in discharge, and help individuals regain independence quickly and safely.
Our Approach
We deliver a rapid-response, outcomes-focused service designed to support recovery, prevent re-admission, and promote wellbeing, using our Digital Care Management Environment (DCME) to coordinate care in real time.
Our team ensures every individual has the right support in place from the moment they step through their front door.
What the Service Includes
Safe Discharge & Settling-In Support
Meeting individuals at home following discharge
Checking the home environment for safety
Ensuring heating, lighting, and essential utilities are working
Immediate support with mobility, positioning, and fall prevention
Preparing a first meal, drinks, and hydration support
Personal & Practical Care
Assistance with bathing, dressing, toileting, continence care
Medication prompts, administration, and reordering
Meal preparation and nutritional monitoring
Light household tasks to maintain a comfortable environmen
Recovery & Rabblement Focus
Support to rebuild daily living skills
Goal-focused care planning to regain confidence and independence
Working in partnership with physiotherapists, occupational therapists, district nurses, and GPs
Encouraging safe mobility and progressive activity
Emotional & Social Support
Reassurance and companionship
Supporting individuals to adjust after illness or injury
Encouraging engagement with community and wellbeing activities
Preventing Re-Admission
Monitoring wellbeing and early identification of deterioration
Liaising with health professionals when concerns arise
Coordinating follow-up care, clinics, and medication reviews
Supporting individuals to access GP, pharmacist, or community nursing services

Enhanced DBS-checked
Fully trained to Care Certificate and All Wales Induction Framework standards
Skilled in reablement, medication care, mobility support, and post-discharge recovery
Supervised by our Registered Manager, a former nurse and Clinical Associate, ensuring clinical insight and robust governance
