What Is Reablement?

Reablement is a time-limited, goal-focused approach that helps adults regain independence after a period of illness or hospitalisation.
Unlike traditional home care, reablement is about doing with the person, not for them.

Our service is ideal for adults who:

  • Have recently been discharged from hospital

  • Are recovering from illness, surgery, or injury

  • Need short-term support to regain mobility or confidence

  • Have lost skills or strength due to a health setback

Assessment & Goal Setting

Every reablement plan starts with a comprehensive assessment, led by our Registered Manager and Clinical Associate (former nurse).
We work collaboratively with GPs, occupational therapists, and social workers to identify personal goals and measurable outcomes.

Short-Term, Intensive Support

Our trained carers provide practical, focused assistance designed to restore independence quickly — typically over a period of up to six weeks, depending on needs.

Person-Centred Practice

We encourage individuals to take control of their recovery.
Every task — from preparing meals to managing medication — is delivered in a way that promotes skill-building and confidence.

Continuous Review & Digital Tracking

Our DCME (Digital Care Management Environment) records all visits, progress notes, and outcomes in real time, enabling families and professionals to monitor improvements and adapt plans instantly.
