Complex Care
When health needs become complex, families deserve more than basic care, they deserve expert clinical support they can trust.
At CCGA Healthcare, we deliver Complex Care at Home for adults with advanced medical conditions, ensuring safety, dignity, and quality of life in familiar surroundings.
Our clinical leadership, digital care technology, and highly trained team make us one of the UK’s most dependable partners for complex, long-term home-based care.
What Is Complex Care?
Complex care (sometimes called continuing healthcare or CHC support) is specialist support for adults with ongoing medical conditions or significant health needs that require skilled, consistent oversight.
At CCGA, we deliver complex care at home for people who would otherwise require long-term hospital or residential care — giving them the independence and comfort they deserve.
Who We Support
We provide complex care for adults aged 18+ with conditions such as:
Spinal injuries and acquired brain injuries
Tracheostomy management
PEG or NG feeding (enteral nutrition support)
Catheter and stoma care
Epilepsy and seizure management
Degenerative neurological conditions (MS, MND, Parkinson’s, etc.)
Palliative and end-of-life care
Ventilation support
Stroke rehabilitation and mobility limitations
Our Complex Care Model
1. Clinically Led Care Planning
Every complex care package begins with a comprehensive assessment led by our Clinical Associate (former Registered Nurse) and managed through our DCME system.
We liaise directly with hospital discharge teams, GPs, district nurses, and community health professionals to design a safe, individualised care plan.
2. Skilled & Specialist-Trained Carers
All CCGA carers receive advanced clinical training tailored to each client’s needs — including PEG, tracheostomy, catheter care, bowel management, and emergency procedures.
Our staff are fully DBS-checked, CQC-compliant, and supported by clinical supervision and reflective practice.
3. 24-Hour or Scheduled Support
We provide:
Live-in complex care for round-the-clock monitoring
Visiting care at flexible times of day or night
Shared packages coordinated with NHS community nursing teams
Each care plan is built for stability, safety, and comfort.
4. Technology-Enabled Oversight
Through our Digital Care Management Environment (DCME), every care activity — from medication to vital signs — is recorded in real time.
Families and professionals have full visibility of care delivery, outcomes, and alerts, ensuring complete transparency.
Clinical Safety and Governance
Safety is at the heart of complex care delivery.
We operate under a strict Clinical Governance Framework that includes:
Continuous risk assessment and incident monitoring
Medication audits and MAR chart accuracy checks
Regular clinical supervision and competency reviews
Infection control and emergency procedure training
Real-time data tracking via DCME
Alignment with CQC’s Key Lines of Enquiry (Safe, Effective, Caring, Responsive, Well-Led)
This ensures all services remain compliant, accountable, and inspection-ready at all times.
