Supported Living

Supported Living Service for Adults (18+)

Independence is not about doing everything alone, it’s about having the right support to live life on your own terms.

At CCGA Healthcare, our Supported Living Service for Adults (18+) empowers individuals to build confidence, develop daily-living skills, and achieve greater independence while receiving the personalised care and support they need to thrive.

Whether you’re a commissioner, social worker or family member, CCGA provides a trusted, person-centred approach to supported living that is safe, consistent and clinically informed.

What Is Supported Living?

Supported Living enables adults to live in their own homes  whether individually or in shared accommodation — with tailored support from trained professionals.

Our goal is to help every person we support live as independently and meaningfully as possible, while ensuring safety, wellbeing and social inclusion.

This service is ideal for adults who:

  • Have a learning disability or autism

  • Live with a mental health condition

  • Have a physical or sensory disability

  • Need help developing daily-living or social skills

  • Are transitioning to independent living

  • Require community engagement and outreach support

Our Supported Living Model

At CCGA Healthcare, we deliver supported living through a partnership model — where individuals, families, local authorities and health professionals work together to design truly personalised plans.

Our support includes:

  • 24-hour or visiting support depending on assessed needs

  • Help with daily living – cooking, cleaning, shopping, budgeting

  • Medication support and health monitoring

  • Community access – attending appointments, work or education

  • Support to build social connections and prevent isolation

  • Emotional wellbeing and companionship

  • Assistance with tenancy management and life skills

  • Specialist support for autism, learning disabilities and complex needs

Focus on Skills, Outcomes and Community Inclusion

Our supported living approach focuses on helping people:
1. Build and maintain daily-living skills
2. Gain confidence in managing money and routines
3. Improve social interaction and community participation
4. Develop self-advocacy and independence
5. Achieve education, volunteering or employment goals
6. Maintain positive mental health and wellbeing
Each person has measurable outcome goals, reviewed regularly through the DCME platform to track progress

Quality, Safety and Compliance

CCGA Healthcare is a CQC-registered, governance-led provider, delivering services in line with the Care Act 2014, Health and Social Care Act 2008, and local safeguarding frameworks. We operate under a robust Quality Assurance Framework, including:
1. Clinical oversight by our management team
2. Regular audits and supervision
3. Continuous feedback from individuals and families
4. Digital performance data and compliance tracking
Our supported living services are safe, responsive and outcome-driven — ensuring value for both individuals and commissioners.

Areas We Cover

We provide domiciliary care across:
London, Kent, Essex, Surrey, Greater Manchester, Merseyside, Wiltshire, and Somerset, with expansion into surrounding areas.

This localised model means we recruit carers locally, ensuring familiarity with the community and reliable, on-time support.

The Care Quality Commission (CQC) is an independent regulator of health and social care services in England. Being regulated by CQC ensures that care providers, like your domiciliary care and supported living services, meet rigorous standards of quality, safety, and compassion.
